Participant profile

• SDA category on plan; supporting evidence on file.
• Support model — SIL, drop-in, hours per day, overnight.
• Communication preferences and decision supports.

Property

• Building type, bedrooms, overnight-worker room.
• Preferred locations with target radius per location.
• Deal-breakers vs preferences, clearly labelled.

Access & equipment

• Mobility aids in use.
• Fixed equipment needed at the dwelling (hoist, backup power, assistive tech).
• Bathroom, kitchen and bedroom must-haves.

Household

• Alone, or open to sharing; compatible housemate characteristics.
• Visitor and family arrangements.
